Why two openings matter
A cave with one entrance is a dead end, and cautious fish treat it accordingly. Anything that can be cornered inside will hesitate at the mouth and often decline to go in at all, which means the shelter you provided is not doing the job you bought it for.

Sizing the openings to your fish
The risk with any ornament containing holes is a fish that fits in and cannot get out, or one that wedges part way. Measure the widest fish in the tank against the openings, and remember that a fish still growing will be a different size in six months to the one you assessed today.
The rule is that an opening should be either clearly too small to enter or comfortably larger than the fish. The dangerous middle ground is a hole a determined fish can push into, which usually happens at night when nobody is watching. Reassess every time you add a new species.
Placing it well
Put it in the mid-ground rather than against the back glass. Being able to see into the openings means you will notice a fish that has taken up residence and stopped coming out, which is often the first sign that something is wrong.
Bed the base into the substrate so it sits firmly and cannot be tipped. If you have several small fish, a second retreat elsewhere in the tank is worth adding, since one shelter in a community tank becomes something to compete over rather than something everyone benefits from.
Keep it out of the direct filter outflow. Fish use a retreat to rest, and a shelter with a current running through it is one they will use far less than a still one. Somewhere sheltered by planting on at least one side works better than open floor.
